Breast Augmentation

This is one of the most common cosmetic rectification carried out the world over. It is an aesthetic surgical procedure to enhance the size and shape of a woman's breast by inserting an artificial implant behind the breast.

A breast implant is a soft shell or a rubber sac that is generally filled with silicone gel or saline (salt-water)and is available in several different sizes to accommodate different patient's needs and surgeon's preference. The feel of the implant is very close to the feel of the normal breast tissue. Any healthy woman above 18 years of age who is not pregnant or breast feeding can undergo breast augmentation surgery safely.

Breast Augmentation procedure

Breast enlargement or augmentation is performed by making a space behind the breast, either behind or in front of the pectoral muscle and placing a breast implant (prosthesis) into this space. The insertion of implant is carried out via the crease under the breast, through the nipple or through the armpit.

In a relatively simple surgical procedure in which the surgeon makes a small incision, through that incision creates a pocket either under the breast tissue or under the muscle, inserts the implant, pushes it to the center so that it is under the nipple and sutures the incision. The surgery is individualized in relation to the type and size of the implant used, the anatomical space created and the placement of the scar. The choice will also depend on the patient's bodily shape, her breast shape and size, her chest shape and her expectations.

The breast tissue itself is not operated on. The implant is simply placed behind the breast to give it more projection and fullness.

Breast Augmentation surgery takes just about 2 - 3 hours. The patient can go home the same day or the following day. A successful breast augmentation gives one fuller and better proportioned breasts that enhance ones overall personality and is also a confidence booster.

Recovery

Breast Lift Surgery in India

Breast Lift or Mastopexy is a surgical procedure, slightly different from breast augmentation and sometimes used in conjunction with it, commonly performed in men and in women to reshape sagging or drooping breasts to give them a firm and youthful shape. In this procedure, the areola or the dark skin around the breasts may also be reduced in size, and the nipple and areola repositioned in relation to the new shape of the breast.

Breast lift surgery has proven to be very successful at making the breasts firmer and more shapely and many women find no need to wear a bra any longer as they feel more comfortable in their new looks.

Breast Lift procedure

As stated earlier, this surgery requires removing of excess skin from around the areola, and possibly also from the bottom of the breast, with shifting of the skin of the breast to tighten the skin envelope and insertion of additional volume such as an implant for added projection and smoothing of the skin. In the process, the nipple and areola can be repositioned to a more appropriate position.

The specific breast lift techniques may vary, but they generally fall into two categories:

  • (a) The concentric (or doughnut) mastopexy for women with smaller and less droopy breasts and
  • (b) The more common anchor-shaped mastopexy.

As a general principal, the more tissue that is cut, the more shaping is possible. The incisions and the quantum for the surgery vary, depending on the extent of sagging of the breast and the amount of loose skin around it. Many a times, breast augmentation is performed simultaneously along with breast lift operation. The surgery for breast lift also takes about 2 - 3 hours and may require you to stay in the hospital for a day or two.

The surgeon also measure the breasts, take photographs, and possibly a mammogram (breast x-ray). He will also discuss with the patient the new placement of nipples, and the resizing and repositioning of the areolas (the darker skin around the nipples). A nutritious and balanced diet is recommended before the surgery, and no smoking at least 2 weeks before the surgery.

Recovery period

Recovery is generally very fast in this surgery. There will be some degree of swelling, bruising, soreness and discomfort in your breasts for few days. One is generally sent home either the same day or the next day, wearing a surgical bra over a gauze dressing. The surgical bra will be replaced by a support bra in the next few days which one will be required to wear all the time (day and night) for about a month or so. Daily activities can be resumed in about 2 weeks time. There might be some degree of numbness around the nipple which will also gradually fade away. One is advised to refrain from performing any strenuous activities like participating in sports or lifting heavy objects for about 6 weeks after the operation and one may be able to resume full daily routine in about 3 months time after the operation.

Face Lift

Face lift surgery, also known as Rhytidectomy, as an ani aging procedure, is another popular cosmetic surgical procedure that is used to reshape or contour the lower two-thirds of the face. It reduces the visible signs of aging specially the loss of skin tautness and elasticity, from face and neck by providing tightening of the skin and muscles to improve cheek, jowl, and neck shape. Face Lift can either be done alone or in conjunction with Blepheroplasty (Eyelid Surgery), Rhinoplasty (Nose Reshaping) or a Forehead Lift.

Face Lift Procedure

Face Lift is usually performed under local anesthesia with oral sedation to help you relax during the procedure. It can also be performed under general anesthesia. The procedure of Face Lift may take anywhere from 3 - 5 hours
An incision is made along the hairline just above the ear which extends down along the front edge of the ear, around the earlobe, and then up and behind the ear extending back into the hairline. The intent for making this incision is to keep it in hidden and inconspicuous. The same kind of incision is repeated on the other side of the face as well. The surgeon then separates the skin of the face from its underlying tissue, moving down to the cheek and into the neck area and below the chin. Herafter, the fat deposits over the cheeks and in the neck may either be removed surgically or with Liposuction.

The surgeon will then tighten bands of small facial muscles and tissue that extend up from the shoulder, below the chin, and up and behind the neck to give the face and neck the 'lift'. The excess skin from the edges of the original incision is trimmed, pulls the skin back and re-drapes it on the face, staples or sutures it into place.

The face is then bandaged to avoid 'Wheaton' formation. The next few hours after Face Lift are spend in the recovery room to ensure that there is no bleeding. One is allowed to go home the same or the following day. Stitches in front of the ear are removed a week later, and the rest of the sutures removed within two weeks. After ten to fourteen days, non-strenuous work and light exercise may be resumed and after four to six weeks, more strenuous exercise may be resumed. And one is once again ready to enjoy the new and improved look.

Rhinoplasty Surgery

Rhinoplasty or Nasal surgery is a surgical procedure to refine and reshape the nose, say to reduce the dorsal hump, modify the nasal tip, narrow the nostrils, reduce the width of the nose, change the angle between the nose and the upper lip, and straighten or narrow the nasal bridge.

Patients undergo nasal surgery for one of three major reasons mentioned below:

  • To correct a crooked nose or deformity as a result of injury
  • To improve the difficulty in breathing as a result of nasal passage obstruction
  • For cosmetic reasons (to enhance the shape of their nose).

Rhinoplasty is sometimes also performed in conjunction with a Face Lift procedure to combat aging changes.

Different types of Rhinoplasty procedure

Reduction Rhinoplasty :

This involves removal of bone or cartilage in an effort to change the shape of the nose.

Augmentation Rhinoplasty :

This involves enhancing or augmenting the nose by adding cartilage, bone, or implant material when nasal deficiency (loss of structure) is encountered.

Closed Rhinoplasty :

This surgery improves the contour of the nose by making the incisions inside the nose and removing, shaving or repositioning the cartilage and bone.

Open Rhinoplasty :

This procedure improves the shape of the nose by making the incisions inside the nose along the nostrils and across the columella(vertical strip), and removing, shaving or repositioning the cartilage and bone

Rhinoplasty procedure :

The procedure of Rhinoplasty takes about 1-2 hours. Incisions are made depending on the reshaping job that needs to be performed. The skin of the nose is separated from it's underlying bone and cartilage which are cut and trimmed to obtain the desired results. The skin is then re-draped over the bone and cartilage. Incision(s) is sutured and nose packs are placed to minimize the chances of bleeding.

After care and recovery period :

For the next couple of hours the patient is kept under observation to keep a watch for bleeding or any other post-operative complications. Nasal packing (if used) stays in the nostrils for one to seven days. Any stitches inside the nose dissolve by themselves while any stitches left outside the nose are removed in three to five days. The nasal splint remains in place for seven days to hold the nasal bones in place, as well as help reduce swelling. Swelling and bruising lasts for two to three weeks. One can return to work or school within ten days.


Varicose veins

The problems of varicose veins are one of the most common problems faced by many. Basically, these are twisted, enlarged veins visible near the surface of the skin. They most commonly occur in the legs and ankles. They are caused by faulty valves in the veins and weakened vein walls. In normal course, veins have a one-way valve which keep the blood flowing against gravity toward the heart. In case of faulty valves, blood accumulates and the resultant pressure build up weakens the veins which become enlarged and twisted. They may develop due to genetics, aging process, or from such conditions that enhance pressure on the leg veins, such as being overweight, pregnant or an occupation that necessitates standing for long periods of time.

Varicose veins are usually not a sign of a serious medical problem. However, in some cases, varicose veins can signal a blockage in the deeper veins. This condition, called deep vein thrombosis , requires evaluation and possibly treatment.

Treatment

Self-care measures

Sclerotherapy :

This is used for treating spider veins or individual, small veins or after surgery (ligation and stripping) on larger varicose veins. However, more recent advances now make sclerotherapy an effective option for larger veins.

Laser therapy

has traditionally been used on smaller varicose veins, especially spider veins. Laser and radiofrequency techniques are now treatment options even for larger veins. They use the heat of laser energy to scar and close off varicose veins.

Microphlebectomy, or stab avulsion:

This is a technique in which several tiny incisions are made in the skin through which the varicosed vein is removed.

Radiofrequency treatment

uses radiofrequency energy to close off a large varicose vein.

Liposuction

It is also popularly known as lipoplasty ("fat modeling"), liposculptureor suction lipectomy ("suction-assisted fat removal"). The concept is surprisingly simple. It is a surgical technique that improves the body's contour by removing excess fat from deposits located between the skin and muscle from various sites on the human body. A small stainless steel tube, called a cannula (from the Latin word for reed, tube, cane) is used for this procedure. The cannula is connected to a powerful suction pump and inserted into the fat through small incisions in the skin for fat removal.

Areas affected can range from the abdomen, thighs, buttocks, to the neck, backs of the arms and elsewhere. The amount of fat removed varies by doctor, method, and patient, but is typically less than 10 pounds (5 kg).

According to the American Society for Aesthetic Plastic Surgery liposuction was the most common plastic surgery procedure performed in 2006 with 403,684

Techniques

Liposuction techniques can be further categorized by

  • the volume of fluid injection, and
  • by the mechanism in which the cannula works.

Volume of fluid injection

Wet liposuction

This involves injection of a small amount of fluid, less in volume than the amount of fat to be removed. The fluid generally consists of Lidocaine(local anesthetic),Epinephrine for contracting the blood vessels and thus minimizing bleeding) and a salt solution( for making it saline, like our body fluids). This fluid helps to loosen the fat cells and reduce bruising. The fat cells are then comfortably suctioned out.

Super-wet liposuction

In this method, the volume to be injected is about the same amount as the volume of fat expected to be removed. This gives a resultant better balance hemostasis and potential fluid overload (as with the tumescent technique). The time taken is one to three hours, depending on the size of the treated area(s).

Tumescent liposuction

In this technique, a much larger amount of fluid is injected into the area, perhaps as much as 3 or 4 times the volume of fat to be removed. The increased quantity creates space between the muscle and the fatty tissue, which creates more room for the suction tube (cannula). Due to high volumeof liquid injected, this procedure takes longer than other techniques. The high volumes of fluid and local anesthetic required for this technique have limited its use with larger people.

Currently this technique is considered the safest and most effective.

Mechanism of liposuction

Suction-assisted liposuction (SAL)

This is the standard method of liposuction. In this approach, a small cannula (like a straw) is inserted through a small incision which is attached to a vacuum device. The breaking up the fat cells is done mechanically by the cannula which is then drawn out of the body by suction.

Microcannula liposuction

As the name suggest, these are very small liposuction cannula that makes possible fine, close to surface contouring, sometimes through much smaller incisions.

Ultrasound-assisted liposuction (UAL)

This technique uses ultrasonic wave energy to rupture the fat cells so they are liquefied before removal. A specialized cannula is used which transmits ultrasound vibrations within the body. This is a good choice for working on more fibrous areas, like the upper back, or male breast area,

where the tissue is firm or especially fibrous, or when large amounts of fat are to be removed. . There is slightly less blood loss. However, there appears to be slightly more risk of seromas forming (pockets of fluid). After ultrasonic liposuction, it is often necessary to perform suction-assisted liposuction to remove this fat.

External ultrasound-assisted liposuction (XUAL or EUAL

As the name suggests, this is a type of UAL where the ultrasonic energy is applied from outside the body, through the skin, making the specialized cannula of the UAL procedure unnecessary.

It was developed because surgeons found that in some cases, the UAL method caused skin necrosis (death) and seromas, which are pockets of a pale yellowish fluid from the body, analogous to hematomas (pockets of red blood cells).

At this time however, it is not widely used and studies are not conclusive as to its effectiveness.

Water-assisted liposuction (WAL)

This technique requires using a thin fan-shaped water beam, which loosens the structure of the fat tissue, so that it can be removed by a special cannula. WAL requires less infiltration solution. The cannula movements are very subtle, helped by the water beam. This is a new technique that there is not much published yet in the medical literature. Not approved by the FDA in the US.

Smartlipo (Laser-Assisted Lipolysis)

This system uses a cannula that contains a laser fiber. The laser energy ruptures the fat cells before they are vacuumed through the cannula. The laser's energy also causes the tissue to coagulate and possibly tighten. The treatment may be more suitable for small areas than for large ones, and possible risks include burning.

Special Preparation

In addition to normal precautions, smoking must be avoided for about two months prior to surgery. Nicotine interferes with circulation and can result in loss of tissue.

Recovery

Patients are generally able to return to work between two days and two weeks, depending on the extent of the liposuction. However, the final result will be evident anywhere from one to six months after surgery, although the patient will see noticeable difference much earlier. However, if the patient does not diet and exercise properly, the remaining fat cell neighbors could still enlarge, creating irregularities.

Combined with other procedures

Liposuction and tightening / lifting skin

The removal of quantities of fat from under the skin during liposuction can leave the skin even more loose. In cases of already drooping skin, lifts such as a Rhytidectomy Facelift, Mastopexy Breast Lift, Abdominoplasty Tummy Tuck, or Lower body lift, Thigh Lift, or Buttock Lift are better tools and may include liposuction during surgery to refine the sculpture. simultaneously, SAL is done minimally in the areas of the undermined tissues to minimize further insult to the blood supply

Introduction

This operation resolves problems of change in the area around the eye, which is hit by aging. The procedure can either be done on upper or lower eyelids or both eyelids, depending on surgeon’s advice.

If it is necessary to solve the symptom of aging to the entire face, first we start with the face-lift and neck-lift operation, followed later with the eyelid operation. During this operation we are unable to improve soft mimic wrinkle (laughter-lines). These lines are caused by the influence of mimicry facial muscles and are individual to each person

Purpose:

To remove skin and tissue from the upper and/or lower eyelids in order to get rid of hooded eyelids and/or eye bags.

The treatment:

Classical Method - Through an external cut.

Operation and post-operative care:-

  • Operation is carried out under local anaesthetic (general anaesthetic is also possible).
  • Surgeon makes an incision on lower eye-lid runs from brim of your lid 3-4mm under the eyelashes to the outer edge.
  • Operative incision on upper eye-lid is located to the skin contour of your eye-lid, sometimes it is necessary to exceed the incision border of the outer edge to perfect removal of skin surplus.
  • Immediately after the operation is applied a compress and bandages.
  • Post-operative swelling of the eyelids and subcuticular suffusion is usual.
  • It is recommended to stay relaxed and use the cold compress during the day.
  • The stitches are removed 4-6 days after operation and it is recommended to apply compress during the night for following 10-14 days.
  • Any sport or physical lifting is not recommended for the first 3-4 weeks.
  • After 14 days is possible to start using cosmetics (make-up etc.)

This information represents a general guide for post-operative recovery, but healing times are individual. You will spend 1 night in clinic after operation. You are then recommended to stay for a further 4-6 nights in hotel before you will have stitches removed.

Transconjuctival Blepharoplasty -

LOWER EYELID - Incision is made on the inside of the eyelid.

Transconjuctival blepharoplasty is a cosmetic surgery procedure that is performed to improve the appearance of the eyes in order to create a more youthful look. The transconjuctival blepharoplasty procedure involves removing the excess fat deposits in the lower eyelid through an incision on the inside of the eyelid.

A transconjuctival blepharoplasty procedure may not be appropriate for patients with the following conditions: low tearing, thyroid disease, hypertension, glaucoma, or detached retina.

It is important for patients to have realistic expectations about the transconjuctival blepharoplasty procedure. This procedure will correct the sagging of the lower eyelids caused by excessive fat deposits. Additional procedures may be required if a patient is concerned with dark circles, wrinkles and other physical characteristics they wish to change about the appearance of their eyes and face.

The transconjuctival blepharoplasty procedure takes anywhere from one to three hours. The transconjuctival blepharoplasty procedure itself involves the surgeon making a small incision on the inside of the eyelid so that s/he can tease out the excess fat deposits located below the eyes.

There are several advantages to the transconjuctival blepharoplasty procedure. Because the incision is made on the inside of the eyelid, no scars will be visible following the surgery. The transconjuctival blepharoplasty procedure also has a lower risk of post-operative complications, as compared to more invasive blepharoplasty techniques involving external incisions.

After the transconjuctival blepharoplasty procedure is complete the patient will be sent home with specific care instructions, which are to be carefully followed in order to minimize potential post-operative risks. A patient may be advised to take antibiotics, pain medication, and use cold compresses to ease the pain and swelling that may occur. Patients usually recover two to four weeks after the surgery.   • We apply a plaster of Paris cast to your nose, which remains in place for 7 days. 
  • After the operation you will be laid on your back, with your head in an elevated position, we apply ice packs to your nose.
  • The first day there will be bruising under your eyes.
  • The plaster of Paris remains in place for 7 days, if you break this recommendation, there are some complications (swelling, shift of operative regions and destruction of operative result).
  • Post-operative bruising should disappear in this time, the swelling lasts about 1-3 months.
  • It is very important to avoid physical lifting and bending forward for 1 month.
  • For 3 months avoid sunshine, solariums and sauna.
